Enhanced Care Management
Enhanced Care Management (ECM) is in partnership with local health plans designed to provide systematic coordination of services and comprehensive care management that is community-based, interdisciplinary, and person-centered for individuals with complex medical and social needs. ECM prioritizes enhancing these individuals’ overall health outcomes and quality of life.
Enhanced Care Management is a Medi-Cal benefit.
ECM providers deliver:
- Comprehensive care management
- Care coordination
- Health promotion
- Comprehensive transitional care
- Individual and family support services
- Referrals to community social supports
Enhanced Care Management
Eligibility: You must be enrolled in Health Plan of San Joaquin, Health Net, or Medi-Cal and meet eligibility criteria. Eligible individuals are identified by their health plan. Individuals or their family members can request ECM services by self-referring or asking for information to see if they qualify.
Cost: This service is paid for by Health Plan of San Joaquin, Medi-Cal, and Health Net.
