Lasting Independence & Family Empowerment
This program is a coordinated specialty care program that focuses on empowering and creating hope for our culturally diverse transitional-age youth and adults (14-25 years of age) experiencing early symptoms of psychosis to continue on their life path through effective treatment, support, and connection.
Our program provides education and support to the family and a support network for each youth and adult. Our mobile team can meet with the family at home, school, and additional community locations. Specialized services include outreach, screening and assessment, individual, family, or couples therapy as needed, case management, education and vocational support, social and life skills groups, multi-family groups, and medication services.
LIFE Path goals:
- Education on early psychosis and its identification for families, individuals, and community.
- Stigma reduction.
- Early intervention through intensive support.
- Lasting independence for youth experiencing early symptoms of psychosis.
- Family empowerment through educational and therapeutic support to return to their chosen life path.
LIFE Path
Eligibility: Individuals who reside in Stanislaus County ages 14-25 years of age who are experiencing symptoms of psychosis and meet the early intervention requirement.
Cost: There is no cost. Funding is provided through Stanislaus County Behavioral Health and Recovery Services and Prevention Mental Health Services Act and Early Intervention.
